Thanks to a certain colleague I have even more information to share on war as a public health issue. This "Cost and Consequences of War in Iraq" is very good. While it obviously has an anti-war stance it contains some excellent information on public health:
"Modern warfare is a public health catastrophe which disproportionately harms civilians rather than combatants... A total of 110,000 Iraqi civilians, including 70,000 children under the age of five and 7,000 elderly, died as a result of 'war-induced adverse health effects' caused by the destruction of infrastructure' [in the last Gulf War]."
Some other very good resources to follow:
